Recreational scuba diving and freediving requires good physical and mental health.
There are a few medical conditions which can be hazardous while diving, listed below. Those who have, or are predisposed to, any of these conditions, should be evaluated by a physician.
This Diver Medical Participant Questionnaire provides a basis to determine if you should seek out that evaluation. If you have any concerns about your diving fitness not represented on this form, consult with your physician before diving.
If you are feeling ill, avoid diving. If you think you may have a contagious disease, protect yourself and others by not participating in dive training and/or dive activities.
References to “diving” on this form encompass both recreational scuba diving and freediving. This form is principally designed as an initial medical screen for new divers, but is also appropriate for divers taking continuing education

If you need to revise any of your responses then you might need an evaluation from a doctor in order to verify your suitability.
Based on the responses you provided, a medical evaluation from a doctor is required. Participation in a diving course requires the doctor's approval.
You should download the medical evaluation form using the link below (which contains your responses to the questions you just answered). This form should then be taken to your doctor for them to sign the final declaration regarding your medical fitness for diving. Your doctor may wish to consult the guidance published by the Undersea & Hyperbaric Medical Society in order to make their evaluation of your suitability for diving.
What if I am already on vacation?
Ideally, you should speak to your doctor prior to your trip, however if you are currently on vacation your dive shop should be able to direct you to a local doctor who is able to conduct a diving medical evaluation.
Please remember that your personal doctor is likely to know much more about your specific state of health (particularly with regards to neurological conditions and surgical history). A doctor who doesn't know details of your medical history will need to take a cautious approach and may not be able to clear you for diving. However your personal physician with knowledge of your precise medical history may be able to make a more informed decision and clear you for diving.
