This website was created by Scuba Squad Mexico S de RL de CV which operates Tank-Ha Dive Center in Mexico. We created the website initially to help us process the medical forms for our divers, but we felt it was useful to the community and therefore we allow the use of this service to anyone.

What personal data we collect and why we collect it
If you complete the medical form we will be collecting your responses to these health questions and we will store these responses indefinitely.
You IP address will be stored alongside your data submission for up to 60 minutes. This helps to ensure only you can download the PDF version of your responses.
Your name, birthdate and signature will be stored for up to 60 minutes. This is required for us to be able to embed these within the PDF that you download.
If you do not click “Generate PDF” you IP address, name, birthday and signature are never stored.
If you are an instructor or a facility and you choose to use a customized link (links such as those which can be generated on https://scubamedical.com/link-generator/) then we will store your facility name, instructor name and facility email and IP address indefinitely. We store this information to allow us to contact you in future about changes to the website and other services we might provide. We will NOT sell your data or share it with any third parties.
Cookies
One cookie is set by a website design tool which we use called Elementor. This design tool is running entirely on our server and your data is not being sent elsewhere. The cookie counts how many times you have viewed a page, but we do not do anything with this information. This cookie is required because we need to use Elementor to build our website and Elementor does not give us the option to disable this cookie.
Analytics
We are not using any third party tools to collect analytics about visitors to this website.
We are collecting the anonymous to the medical questions and we may publish the aggregated results to see the total number of medical screening have been performed and which areas of the screening process required the diver to seek a medical evaluation from a doctor.
Who we share your data with
If you were sent here using a special link then during the medical screening process you will be given the option to send a PDF copy of your responses and your name, birthdate, signature to the dive center that sent you to this website. If you choose to do this, then your personal information will be shared with that dive center in the form of a PDF document attached to an email.
How long we retain your data

What rights you have over your data
Although we promise not to retain your data for more than 60 minutes, in reality your data is often delete much quicker than this. Therefore it is not possible for us to provide you with an option of downloading data you have submitted through this website because we have no method of identifying your particular submission.
Similarly, once your personal data has been automatically scrubbed from our servers, it is no longer possible for us to locate your medical submissions for the purpose of deleting the submission values.
If you are an instructor or a facility and you choose to use a customized link to send people to this website, we will retain your facility name, instructor name, facility email and IP address. If you would like a copy of this information or you would like us to delete this information, send us an email to info@scubamedical.com
Where we send your data
Your personal information is held on servers managed by Amazon Web Services, located in USA for a maximum of 24 hours. Personal information is purged from the servers by an automated process and this process also runs prior to any backups being taken (therefore your data is never included in a backup).
This website runs on a web server located in the USA and owned by Amazon Web Services (AWS). The owner of Scuba Squad Mexico S de RL de CV has a contract with AWS which includes the AWS privacy shield. The privacy shield is in place to ensure data from EU citizens is handled correctly.
