What is ScubaMedical?

ScubaMedical.com is designed to guide you through the Diver Medical Participant Questionnaire that was published by the Undersea & Hyperbaric Medical Society (UHMS) in June 2020.

ScubaMedical makes it easy to follow the intricate instructions and assessment criteria, and provides a quick, straightforward way to find and complete the medical screening questions online.

How it works

  1. Answer the medical screening questions online — the tool guides you through each section of the official form.
  2. See your result instantly — find out whether you appear eligible or whether a physician evaluation is recommended.
  3. Download or share a PDF — generate a completed copy of the official form to keep for your records, take to a physician, or send directly to your dive center through the portal.

The medical screening tool

The Medical Screening Tool was produced by the Diver Medical Screen Committee (DMSC) with significant input and partnership from the Recreational Scuba Training Council (RSTC) and Divers Alert Network (DAN).

For dive centers & instructors

ScubaMedical includes a free Company Portal that lets dive centers and instructors manage medical screenings for their clients. Through the portal you can:

  • Create a branded public link to share with your participants
  • Customize welcome messages, pass/fail text, and PDF details
  • Review, download, and export participant submissions
  • View anonymized statistical trends across your submissions

Registration is free and takes a couple of minutes. Register your dive center to get started. Once registered, you can share your personalized link with clients so their completed medical forms are submitted directly to your portal.

Resources for physicians

When a diver's responses indicate that further medical evaluation is needed, they are directed to visit a physician. Physicians who are unfamiliar with dive-medicine fitness standards can consult the guidance published by UHMS, which includes detailed clinical criteria for each screening question. The UHMS site also provides additional resources for doctors evaluating diving fitness.
